ASSESSMENT

For this assessment, you are required to provide a report and documentation that demonstrates your ability to assist with actions and activities in the response to a workplace incident. You may use the scenario below as a case study, or alternately, you may have assisted during in a workplace incident and may use that situation as your case study. During the assessment, your trainer will assess and observe your skills and aptitude and assist you to develop these to effectively assist in a workplace emergency. ENSURE prior approval from your trainer has been obtained for you to use a current workplace situation.

Scenario:

You work for a BOB DISTRIBUTION COMPANY that has an office and warehouse at 26 BobbitySt, Bonville 3333. Employees park in an area at the rear of the building and office staff walkthrough the building to the office area. Until recently all loading and unloading of goods was done manually using a pallet jack. The company recently purchased a new forklift on the first Friday of this month. It is being driven by one of three people in the store, all of whom completed the required nationally recognised course to gain an appropriate forklift licence. One of the office workers, Jacinta Allan, was struck by the reversing forklift as she entered the building on Monday of this week at 8:30am. She required hospitalisation as her left leg was broken. You have been asked to assist in a WHS investigation of an incident because you were a witness and rendered assistance prior to the worker being transported to hospital. The general manager, being aware that you are currently undertaking studies in WHS, requests your assistance in the incident investigation. He has assigned the HR manager to ensure that the incident is investigated in a systematic manner and the HR manager has asked you to provide Investigation Report of the incident response, you are able provide further details because you also rendered assistance prior to the worker being transported to hospital.

Your report should cover:

1. Gather all relevant WHS data and information, relevant to the legislative framework your organisation operates within and present in a table format. Identify stakeholders and include this in your introduction.

2. Provide a brief outline of the incident, and explanation of relevant workplace policies and procedures that were followed prior to, during and following the incident including all internal and external notification details.

3. Using documented industry specific first aid guidelines. Include in your report 3 injury’s that were or may need to be managed, at the incident site and how those injuries/illnesses were managed.

Attach documented evidence - ‘Injury register’ to your Report. Ensure you maintain confidentiality of any personal information.

4. (a) Explain how information was gathered from and provided to the following persons. The injured person, the supervisor and the HSR.

(b) List additional sources of data and information (2 internal and 2 external) that identify situations leading up to the incident?5. Provide further relevant information for at least 5 of the following dot points addressed individually.

• Exact bodily location of the injury.

• Detail of the force or cause of the sustained injury.

• Sequence of events.

• The nature of plant and equipment that was involved, including guarding.

• The requirement for PPE if any.

• Detail of the work environment, photos, sketched.

• Details of the relevant work methods, workflow, storage systems, documentation, meeting minutes, measures used to preserve the incident site for WorkSafe, etc.

• Have witness name s been listed and documented, where has the information been documented.

• Was the injured worker able to return to work following the injury?

• Has a ‘Worker’s injury claim form’ been received by the organisation?

6. Provide information in your report about the actions that will need to be taken to prevent are occurrence of the incident. Cite industry requirements or best practice for the safety of workers exposed to similar risks identified in the incident (your answer must include at least one OF THE FOLLOWING – a Regulation, Code, or Guidance material)

7. Using the Hierarchy of Controls, detail 2 possible corrective actions to prevent future similar incidents occurring. Each recommendation must include at least 2 levels from the Hierarchy of Controls. (2x2 plus 2x2 examples).

8. Complete an Incident Investigation form and an Incident Notification form and attach to your report as further evidence of incident, illness, injury review. Ensure that these forms are accurately completed and legible.

9. Develop a communication plan to alert all workers to any changes in the workplace, eventuating from your investigation. What will be communicated? How will it be communicated? When will it be communicated? Who will be responsible? This information should be provided in a table format.
