Membership Application Form
You have been asked to create a membership application form. You have also been given the information and features that the form is to contain.
The form requires the following settings:
• Restricted Editing – for Filling in Forms
• To be saved as a file type of Word Template (.dotx)
It must include the following form controls (as indicated on the draft):
• Drop down lists
• Check box fields
• Text fields
• Date picker fields
Include the Company’s name, logo (landcare.png – found on the OLS) and purpose of the form.
You will design this form so that it is easy to understand and simple to complete.
Your form will collect the following information:
Applicant’s details such as: (most fields could be text fields)
Title (for example Mr, Mrs, Ms etc) (a drop down field would be appropriate)

State (use a drop down field with the list of Australian states)

Date of Birth: (Insert a date picker)
Subscribe to newsletter: (Insert a check box)
Preferred executive role: (Insert a drop list with President, Treasurer and Secretary options)
Membership Approved: Yes/No (check box field)
Additional Comments:
Volunteering availability: (Insert check boxes, for example list the days of the week)
